VAS Pain Score Converter

How it is calculated

The conversion is a straight division: NRS = VAS / 10, where VAS is the distance in millimetres from the "no pain" end of a 100 mm line to the patient's mark. The NRS result is shown to one decimal place. The tool then assigns a category from the NRS value: exactly 0 is None, above 0 up to and including 3.0 is Mild, above 3.0 up to 6.0 is Moderate, and anything above 6.0 is Severe. So a VAS of 30 mm is still Mild and 31 mm is already Moderate. It also echoes the score as a percentage of the line. Only values from 0 to 100 are accepted; anything outside that range returns no result. The category bands are a simple fixed cut-off in the tool, not a diagnosis.

Worked example

A patient marks the 100 mm line 45 mm from the "no pain" end, so VAS = 45. Dividing by 10 gives 4.5 on the 0-10 numeric scale. Because 4.5 is above 3.0 and not above 6.0, the category is Moderate. The tool also reports the score as 45% of the line. A later mark at 28 mm would be 2.8, which falls in the Mild band, so the change is easy to compare across visits.

Common mistakes

  • Entering a score from a 0-10 scale as if it were a VAS. A reported "6" typed here means 6 mm, which converts to 0.6 and is shown as Mild.
  • Measuring on a line that is not exactly 100 mm, for example a printed or on-screen scale that was resized. Rescale first: 5 cm on a 10 cm line is 50 mm VAS only after you convert it.
  • Typing centimetres. 4.5 cm must be entered as 45, not 4.5.
  • Treating the category as a clinical grade. The tool applies fixed cut-offs of 3 and 6, so 3.1 versus 3.0 flips the label, and other protocols use different bands.
  • Comparing one reading with another from a different patient or scale type. The conversion is arithmetic only and says nothing about how the patient interpreted the line.

About this calculator

The Visual Analog Scale (VAS) measures pain by having a patient mark a point on a 100 mm line ranging from “no pain” to “worst pain imaginable,” giving a score from 0 to 100. Because many clinical records, research studies, and pain-management protocols instead use the simpler 0-10 Numeric Rating Scale (NRS), this calculator converts a VAS millimeter score to its NRS equivalent (dividing by 10) and classifies it into standard pain categories such as mild, moderate, or severe.

Both scales are widely used pain-assessment tools in nursing, anesthesiology, and pain-management medicine, and being able to convert between them matters when comparing pain scores recorded in different formats across visits, studies, or hospital systems — a VAS score from a research paper, for instance, often needs to be translated to NRS to match a hospital's standard charting scale. The mild, moderate and severe bands are a simple fixed convention (up to 3, above 3 to 6, above 6). Published cut-offs vary: in chronic musculoskeletal pain, Boonstra et al. (2016) found 5 or less mild, 6–7 moderate and 8 or more severe on the 0–10 scale, and 3 or less, 4–6 and 7 or more in patients with a low tendency to catastrophize.
