Vancomycin AUC Dosing Calculator
Enter the target AUC24 and the patient's creatinine clearance to estimate the daily vancomycin dose.

How it is calculated
The tool uses a one-line clearance model. Vancomycin clearance is estimated as CL (L/h) = 0.75 × CrCl × 0.06, where CrCl is creatinine clearance in mL/min (the factor 0.06 converts mL/min to L/h, and 0.75 is the fixed share of creatinine clearance assumed to apply to vancomycin). At steady state AUC24 = daily dose / CL, so daily dose (mg) = target AUC24 (mg·h/L) × CL. Half of that is shown as the dose every 12 hours, rounded to the nearest 250 mg. Both inputs must be above zero. There is no weight, age, loading dose or measured level in the model, so this is a rough starting estimate and not a dosing instruction. Local protocols decide the real regimen.
Worked example
Target AUC24 of 500 mg·h/L and a creatinine clearance of 90 mL/min. Clearance: 0.75 × 90 × 0.06 = 4.05 L/h. Daily dose: 500 × 4.05 = 2,025 mg, which the tool shows as 2,025 mg. Dividing by two gives 1,012.5 mg every 12 hours, and rounding to the nearest 250 mg gives 1,000 mg. So the estimate is about 1,000 mg twice daily, to be checked against measured levels and the local protocol.
Common mistakes
- Entering creatinine clearance in mL/s, in L/h or as the serum creatinine itself. The model expects CrCl in mL/min, so 90 means 90 mL/min.
- Using the 12-hour dose as a final order. It is rounded to 250 mg and based on a single fixed clearance relation, so it needs adjustment from measured levels and the patient's condition.
- Mixing up the target. The AUC24 is the area over a full day in mg·h/L; entering a trough concentration or a 12-hour area will give a wrong daily dose.
- Expecting the tool to include a loading dose, weight-based dosing or a dosing-interval choice. It returns only a daily dose, a 12-hourly share and the clearance.
- Using a creatinine clearance from a patient whose kidney function is changing fast. Steady state is assumed, so in unstable kidney function the estimate can be misleading.
About this calculator
Modern vancomycin dosing guidelines (ASHP/IDSA/PIDS/SIDP 2020) moved away from trough-only monitoring toward AUC24-guided dosing, targeting an area-under-the-curve of roughly 400-600 mg·h/L over 24 hours — high enough for antibacterial efficacy against MRSA, low enough to limit nephrotoxicity risk.
Using a first-order pharmacokinetic clearance model, the daily dose can be estimated from the target AUC24 and the patient's creatinine clearance (which drives how fast vancomycin is eliminated). This calculator is a quick reference for clinical pharmacists and prescribers working through the AUC-based dosing approach — it is not a substitute for institutional protocols, therapeutic drug monitoring, or clinical judgment, and is not medical advice.
