Antral Follicle Count Calculator

Enter the antral follicle count for each ovary to get the total AFC and reserve band.

Result

14 follicles
Normal (7–20)
6 left + 8 right

How it is calculated

The tool adds the two counts you read from the ultrasound report: total AFC = left ovary + right ovary. It then places the total in a band using fixed cut-offs: under 7 is Low, 7 to 20 inclusive is Normal, and above 20 is High (possible PCOS). Both counts must be whole numbers of zero or more; a decimal or negative entry gives no result. The bands are one simple scheme built into the tool. Laboratories and clinics use different thresholds depending on age and method, so a category here is a rough orientation, not a diagnosis, and it says nothing about AMH, age or cycle regularity. The result is not medical advice.

Worked example

An ultrasound report lists 6 antral follicles in the left ovary and 8 in the right ovary. Total: 6 + 8 = 14 follicles. Check the bands: 14 is not below 7 and not above 20, so the category is Normal (7–20). The tool also repeats the split as 6 left + 8 right. If the counts had been 3 and 3, the total of 6 would fall under 7 and the category would read Low (<7). Your clinician interprets the count together with age and hormone tests.

Common mistakes

  • Entering the total in one box and zero in the other. It still sums correctly but you lose the left and right split; enter each ovary separately.
  • Typing the follicle size in millimetres instead of the count, or a range such as 5-8. Only whole numbers are accepted.
  • Treating the band as a diagnosis. The 7 and 20 cut-offs here are the tool's own, and a report or clinic may define low and high differently.
  • Comparing counts from different cycle days or different scans as one figure; one count reflects one scan.
  • Reading "High" as confirmation of PCOS. A high count is one sign among several and needs a clinician's assessment.

About this calculator

An antral follicle count (AFC) is an ultrasound count of the small resting follicles (2–10mm) visible in each ovary at the start of a menstrual cycle, and it's one of the main markers doctors use to estimate a woman's remaining ovarian reserve. This calculator simply adds the left and right ovary counts to get total AFC and places it into a reserve category.

The categories used here are: a total AFC under 7 is shown as low, 7–20 as normal, and above 20 as high (which can flag polycystic ovarian morphology) — though exact cutoffs vary by lab and age. AFC is measured by transvaginal ultrasound, usually on cycle day 2–5.

Reproductive endocrinologists and fertility specialists use AFC alongside AMH (anti-Müllerian hormone) blood levels to predict how a patient will respond to ovarian stimulation in an IVF cycle — a higher count generally means more eggs retrieved. This tool is for reference only, not medical advice.
